Find your physician directory profile, go to:
www.choc.org/findadoc
1) Enter in last name & click “find physician”
2) Check profile (location, specialty, education, languages, board cert., etc.)
3) Check photo (take new photo or schedule photo shoot, if photo is older than 3 years)
