In the spirit of collaboration and based on feedback from many of our community partners, we’ve created the following tools to make referring patients to CHOC as efficient and convenient as possible. Along with our redesigned CHOC.org website that is now mobile-responsive, we have improved the referrals content area. We have created four referral sections designed to help providers navigate their way through the referral process. Each section also features detailed instructions.
Locate information about CHOC pediatric emergency department and the pre-arrival number to be connected with the on-call emergency physician.
Locate information about the hospital admitting process and hospital services directories for both CHOC Hospital in Orange and CHOC at Mission Hospital in Mission Viejo.
Locate comprehensive information for highly-referred specialties, including referral guidelines, referral request forms, practice profiles, provider directories, location directories and instructions for sending a virtual fax. In addition, referring providers can also access information about our physician concierge service which expedites physician-to-physician communication with CHOC specialists.
